Waiver

I give my child(ren) permission to participate in the Kesher School's Out Day provided by Temple Sinai on Friday, April 3, 2026. I understand that participation is voluntary and that the Temple Sinai staff do not assume liability in the case of accident or injury.

I understand that an attempt will be made to locate me in case of emergency or accident, but I hereby authorize Temple Sinai staff to call a physician or seek emergency room treatment as necessary for my child, in case of an emergency, and agree to pay all expenses incurred (this authorization is given pursuant to the provisions of section #25.8 of the Civil Code of the State of California).
